1. Identity of the Data Controller

As Uzm. Dt. Hülya Çetin Çelik ("the Dentist"), we attach great importance to the security and confidentiality of your personal data in our capacity as data controller, in accordance with the Personal Data Protection Law No. 6698 ("KVKK") and other relevant legislation.

2. Personal Data Processed and Special Categories of Personal Data

Through the contact form on our website or redirected communication channels (phone, WhatsApp, DoktorTakvimi, etc.);

  • Your identity data (name, surname),

  • Your contact data (phone number, email address),

  • Your special categories of personal data relating to your dental and oral health history, requests, or treatment processes (your health data),

are processed within the framework of legal compliance.

3. Purposes and Legal Basis of Data Processing

Your personal data are processed in accordance with Articles 5 and 6 of the KVKK for the purposes of scheduling appointments and treatment plans, answering your questions, carrying out information activities, and fulfilling legal obligations arising from the regulations of the Ministry of Health.

4. Transfer and Retention of Data

Your personal data are protected by technical and administrative measures to prevent unlawful access. Except for legal obligations and authorized public institutions, they are not transferred to third parties. Once the purpose of processing ceases to exist and the legal retention periods expire, your data are destroyed ex officio or upon your request.

5. Your Rights

You may submit your requests regarding your rights under Article 11 of the KVKK in writing to Konak Mah. 1. Badem Sk. B Blok No:26, Lotus Office Door No:82, Nilüfer / Bursa.
